Well it's a typing game. You type the commands to do something. I'll give you an example: Take gloves, use gloves. Type "look" for more help on finding items.

Type deliver at the title screen, type play while you are already playing. Those are two other medals.
I can't get the one for beating it in 20 seconds
You have to be quick bout it. I mean, I was so fast, I didn't realize I got the trophy until I did it like 3 more times.
